Lasik and other corrective eye surgeries are considered to be 'cosmetic surgery'; you will not be eligible for subsidies nor can you use your Medisave, unless the surgery is done for 'medical reasons' e.g. you are visually handicapped and lasik is the only option left.

The only hospital I know that does corrective eye surgery is the Singapore National Eye Centre (SNEC), next to the Singapore General Hospital. I am very sure they are not the only institution in Singapore that does it but I do not know of any other hospitals.

Cost varies depending on which procedure you elect to have performed on you and the most appropriate procedure is again determined by the condition of your eyes. SNEC has a pre-operation consultation where your eyes will be subjected to a battery of tests to determine their suitability for corrective eye surgery, as well as a talk with a counsellor to ensure you know the details and risks of the surgery.
References :
Singapore National Eye Centre: http://www.snec.com.sg
